If its an organic post, here are a few things I’d recommend double-checking:
Instagram permissions You already mentioned you refreshed permissions, which is usually a quick fix, but since it didn’t work in your case, we can rule that out for now.
Check for platform-wide issues Sometimes this is related to a temporary issue on Instagram or ManyChat’s side. You can check the current system status here: https://status.manychat.com/
One-time trigger behavior Just as a reminder, Instagram comment automations only trigger once per user per post. If the same user commented again, the automation would not fire a second time.

Hi, the issue has not been resolved yet. I have been having this issue since February 14th, and for the past few days it has gotten to a point where my page is critically affected operationally. My DM automations are a core part of my lead capture and customer communication process, and they are currently unreliable. I have tried the steps you have given, none gave any solutions to this problem
If the “You messaged…” notice appears but no DM is delivered, the trigger is firing but Instagram is blocking the actual message.
This is usually a Meta side restriction or rate limit, especially if you send high DM volume. ManyChat will not always show an error.
Check Account Quality and messaging limits in Meta Business Suite. Then test with a brand new post and a very simple text only flow. If it still fails, you will need to contact ManyChat support with timestamps so they can review delivery logs.

How can i contact the ManyChat support? I have sent a ticket through the automated helper bot on Thursday, but had no feedback yet. How long does it usually take?

Instagram Comment-to-DM fires but no DM is sent because Meta may block messages due to restrictions or rate limits. Check account quality and messaging limits in Meta Business Suite. Test with a new post and simple flow. If it still fails, contact ManyChat support here: https://help.manychat.com/hc/en-us/p/get_support_page
